What the Latest Silicone, Gummy Bear, and Structured Implants Mean for You

Breast implants have changed quite a bit since the decision came down to “saline or silicone?” The familiar round shells are still here, but their internal engineering has become far more advanced. Silicone gels now come in different levels of firmness and shape retention, and saline implants have gained internal structures that help control fluid movement. Surgeons also have a broader range of widths, projections, and profiles, allowing for far more precision than the old “pick a cup size” conversation.

Silicone Gel Implants Have Gotten an Upgrade

Silicone breast implants contain gel inside a flexible silicone outer shell. Earlier generations used softer gel with more fluid movement, but today’s cohesive silicone gel holds together with greater consistency while retaining the softness that draws many patients to silicone implants.

Cohesivity runs on a spectrum. Softer gel tends to move with the breast, while firmer gel provides greater control over breast shape and may reduce folding or visible rippling. Patients with less natural breast tissue may benefit from that added structure, although a firm implant can feel more noticeable when tissue coverage is limited.

Then come the other choices: implant width, projection, profile, shell, placement, and volume. Two silicone gel implants with the same number of cubic centimeters can create very different silhouettes. One may sit wide and low-key across the chest. Another may direct more volume forward. Implant size alone can’t explain what either device will do.

What is a Gummy Bear Implant?

“Gummy bear” may be the most charming nickname in breast surgery. It refers to form-stable implants filled with cohesive silicone gel that tends to hold together when cut. Despite the candy comparison, gummy bear breast implants don’t feel like chewy sweets beneath the skin.

Different gummy bear implants have different levels of firmness. Some highly cohesive implants create strong shape retention and defined upper-breast fullness, while others allow more natural movement. Round implants can contain cohesive gel, too, so the gummy bear label doesn’t mean the implant has to be teardrop-shaped.

Because silicone gel-filled breast implants arrive prefilled, placement may require a slightly longer incision than a saline implant. Firmer gel may also make the edges easier to feel in patients with thin breast skin. Dr. Walters weighs those tradeoffs against the shape control and reduced rippling that cohesive gel can provide.

Saline Implants Are Still An Option

Saline breast implants have a silicone outer shell filled with sterile salt water. They’re inserted empty and filled during breast implant surgery, which may allow for a smaller incision and minor volume adjustments between the breasts.

A saline implant also makes rupture easy to recognize. If the implant shell fails, the saline is absorbed by the body and the breast loses volume. A ruptured silicone implant may cause no immediate change in appearance, which is why the issue is often called a silent rupture.

Traditional saline-filled breast implants can feel firmer and show more rippling, especially when there’s limited breast tissue covering the device. Placement beneath the chest muscle may provide added coverage in certain patients. Your chest width, tissue thickness, activity level, body type, and preferred breast shape all help determine if saline makes sense.

Implant Selection Has Become a Geometry Problem

The many types of breast implants can look similar in photos, yet small changes in their dimensions can have a major influence on the final breast augmentation result. Dr. Walters considers the width of your chest, the base of each breast, skin elasticity, tissue coverage, implant projection, and how much weight your breast skin can support.

This becomes useful when you want fullness without a breast size that overwhelms your frame. A narrow, high-projection implant concentrates volume forward. A wider implant spreads that same volume across more of the chest. Both can look beautiful on the right body and wildly out of place on the wrong one.

Skin position changes the breast implant procedure, too. An implant adds volume, while a breast lift raises and reshapes breast tissue. When stretched skin or a low nipple position is part of the concern, combining procedures may create a better shape than relying on a larger implant to fill the loose skin.

Your Implants Will Need Check-Ins

Breast implants are long-term devices, though they aren’t lifetime devices. There’s no universal rule requiring replacement at the ten-year mark. An intact implant that feels comfortable and looks good may remain in place, with monitoring based on its type and your plastic surgeon’s recommendations.

Revision surgeries may become necessary for implant rupture, capsular contracture, pain, displacement, asymmetry, or a change in personal preference. Pregnancy, weight fluctuation, aging, and changes in natural breast tissue may also alter the result even when the permanent implant remains intact.

Current FDA recommendations advise ultrasound or MRI screening for silicone gel implants five to six years after placement, followed by imaging every two to three years. Saline deflation tends to announce itself through visible volume loss.

Routine breast cancer screening continues after breast augmentation. Tell the imaging center about your implants before a mammogram so the technologist can use views that show more breast tissue.

Implant Surfaces Deserve a Conversation

Most implants used for cosmetic breast augmentation in the United States now have smooth surfaces. Textured implants were developed to encourage surrounding tissue to grip the shell, but textured surfaces have received increased attention because of their association with breast implant-associated anaplastic large cell lymphoma, or BIA-ALCL.

BIA-ALCL is an uncommon immune-system cancer found in fluid or scar tissue around an implant. According to the FDA, it has occurred most often in patients with textured implant shells.

New swelling, persistent pain, fluid collection, or a mass that develops years after breast implant surgery should be evaluated. Keep the device card provided after surgery so you have a record of the implant manufacturer, model, size, and surface.

Choose the Implant That Makes Sense Beyond Surgery Day

Patients often arrive with an implant name they found during late-night research. Sometimes it fits. Sometimes another gel firmness, width, profile, or fill creates the look they had in mind with better support for the breast tissue.
